What to Bring
- Driver’s license or photo ID
- Health insurance card
- Name and address of your primary care physician
- Relevant x-rays and/or test results
- Current medication list
- Names and addresses of other physicians to whom we should send a report
- Names and phone numbers of your preferred pharmacy and medical equipment company
What to expect at your appointment:
After checking in with Reception, a nurse will record your height, weight, blood pressure and pulse. They will then review your medical history, asking questions about your current symptoms, medications and allergies, as well as your family, social, and occupational circumstances. In some instances, a respiratory therapist will test your lung function.
The physician will then review your records and interview you further about your symptoms and concerns before performing a physical exam. The physician will assess your condition and recommend treatment or further testing. A report will be sent to your primary care provider as well as any additional physicians that you designate.

If you need to refill a prescription prior to your next appointment, please call your pharmacy. The pharmacy will contact our office for you.
