Gray top section: The average initial consultation lasts 90-120 minutes; extra time may be required for consultation with your primary care clinic. Preparing a list of your questions and concerns before each visit will help ensure that your needs are addressed. Please note that many of our patients have asthma and are especially sensitive to fragrances. Kindly avoid the use of perfume and scented lotions when visiting our clinics.

Driver’s license or photo ID
Health insurance card
Name and address of your primary care physician
Relevant x-rays and/or test results
Current medication list
Names and addresses of other physicians to whom we should send a report
Names and phone numbers of your preferred pharmacy and medical equipment company

Before your initial appointment, you will be asked to complete several forms. You can download the forms or we can mail them to you upon request. Please bring all completed forms with you to your appointment.

What to expect at your appointment:

After checking in with Reception, a nurse will record your height, weight, blood pressure and pulse. They will then review your medical history, asking questions about your current symptoms, medications and allergies, as well as your family, social, and occupational circumstances. In some instances, a respiratory therapist will test your lung function.

The physician will then review your records and interview you further about your symptoms and concerns before performing a physical exam. The physician will assess your condition and recommend treatment or further testing. A report will be sent to your primary care provider as well as any additional physicians that you designate.
